I have an old Microsoft Sidewinder game pad, PC GAMEPORT version (DB15 male connector on end of cable). Can this be adapted (via use of dupont cables for testing purposes) to fit the male DB15 on a NEO GEO controller port?

When looking at a pinout for a standard PC gameport, it doesn't look like there are individual pins for the 4 directions on the D-button (only one pin for both X directions and one pin for both Y directions, with this scheme duplicated on other pins but used for player two only) like what the NEO GEO port specification has.

No. A PC joystick or gamepad is a poor choice to try an adapt to a neo geo style input. Furthermore microsoft sidewinder joysticks and game pads used funky undocumented tricks to support extra buttons and bidirectional digital communication through the 15 pin PC game port. ie. hijacking the MIDI in/out pins (usially on pins 12 & 15) for use as a serial port.

Because X and Y are analog inputs.
